Male & Female Infertility
Infertility in males and females due to any cause, lack of sperms, urinary tract hardening, reduced male potency, closure of fallopian tubes, weakening of the uterus, lack of ovulation, ovarian cysts (PCOD), hormonal Inequality, abnormality of menstruation and its connected problems, leucorrhoea, blood leucorrhoea
Fertility in women
Each month a fertile woman releases an egg and her body prepares to fertilize it. Each egg is enclosed in a special set of cells called follicles that help the egg grow and develop. In a normal cycle, only one egg will grow to maturity and be released from its follicle. That is why so many pregnancies end in miscarriage. As the egg grows, the lining of the uterus begins to thicken. This is a very special place where the first embryo will adhere and never end. About 14 days after the onset of menopause, the mature egg is released (ovulation). This is the best time to have sex and try to have a baby. The egg enters the fallopian tubes where, in about 24 hours, it can be fertilized. When a fertilized egg begins to develop into an embryo,
it passes through the fallopian tube into the uterus where it can be implanted. If the egg is not fertilized, or the embryo does not develop, all the extra lining of the uterus prepared by the embryo breaks down and causes menstruation.
Complications may occur as a result of the failure of the egg, due to obstruction of the fallopian tubes, or problems with the lining of the uterus.
Fertility in men
Sperm level and amount of sperm both play a major role in fertility. Some men produce low levels of sperm (oligozoospermia) which reduces the chances of sperm reaching the egg.
Reproductive problems can also be caused by inability to swim properly, or abnormal sperm formation.
All men who want to be treated for infertility will be asked to go to the clinic for a sperm test, which will check all of these things.
Sperm problems may be the cause of infertility in an estimated 50% of couples who suffer from infertility.
If sperm problems are identified, we can provide treatment for pregnancy using ICSI (intracytoplasmic sperm injection).
Some men produce sterile sperm. This is called azoospermia. The male part of azoospermic has normal sperm production from the testicles, but there is a blockage that prevents sperm from entering the sperm.
This is Due to:
- Failure of sperm transport tubes to grow
- Closure of the fallopian tubes
- Previous vasectomy surgery
All of these can be treated using IVF with ICSI after sperm recovery using minimally invasive surgery.
Expand our service to provide a specialized male reproductive clinic with a multidisciplinary team that includes andrology and urology technologies
How you can improve your own chances
Whether you are trying to get pregnant without our help, or just want to improve your chances of getting pregnant, there are a few things you can do:
- Eat a healthy diet and get proper exercise – obesity has been shown to have a negative effect on fertility. You can seek further advice on this from us or your doctor.
- Don’t smoke – this will not only reduce your chances of getting pregnant but any parent smoking will affect your unborn baby.
- Avoid alcohol abuse.
- Be aware of which day you may be ovulating or in general we would recommend that you try to have sex at least twice a week which should mean that you will always have sex during fertilization.
- Avoid using vaginal lubricants such as KY Jelly, Durex Play or Astroglide that are toxic to the penis and will reduce your chances of getting pregnant.
- Women should take folic acid pills in preparation for pregnancy to reduce the risk of your baby developing developmental problems. Some women need a high dose of folic acid (5mg). Women with an indication of weight gain of 30 or more, epilepsy, diabetes, or malabsorption conditions such as inflammatory bowel disease require this dose.
Obviously, most people will achieve all of the above and still have difficulty conceiving. If you are trying to get pregnant and are unsuccessful, then this is the stage where you should seek additional help and advice.
Fortune Healthcare Fertility Center offers the latest fertility tests available to our patients.
We offer the latest fertility tests available to our patients. Our team understands how difficult it can be for you to choose additional tests for your treatment. We will guide you to fully understand why screening may help you to be more successful in treatment.
What tests do we offer?
We are using an injectable test called ENDOMETRIO to help us personalize your embryonic transplant. We can also do research on sperm DNA to see if there is any damage, possibly causing reproduction. Our pre-transplant genetic clinics mean that we can also give you an embryo test to give you the best chance of getting pregnant with you.
1.In Vitro Fertilisation (IVF)
IVF is suitable for:
- Unexplained infertility
- Tubal Damage
- Male factor problems
- Ovulation disorders
- Older women
What is it?
IVF literally means fertilisation ‘in glass’ hence the familiar name of ‘test-tube baby ‘. Eggs are removed from the ovaries and fertilised by sperm in a laboratory dish and the resulting embryos
What does it involve?
IVF involves taking birth control pills tailored to your individual needs. The production of your hormones is temporarily halted and your ovaries are stimulated to produce more eggs than usual. Egg production is monitored by routine ultrasound examination. When the eggs are ready to hatch, you will have a little work to collect (egg collection)
You will be asked to produce a sperm sample at the clinic on the day of egg collection. The sperm are collected in a sample pot and after a while, it is washed and filtered at high speed to select the best sperm mixed with the eggs. If for any reason it is not possible for you to produce a sample per day, the sperm may be frozen in advance.
What happens next?
Eggs are mixed with sperm in the laboratory and left to fertilize. After two or more days, one or two embryos implant in your womb. Any good quality embryos can be frozen and stored for future use.
How effective is it?
Success rates depend on many factors, but most importantly the age of the women. Opportunity does not improve as each obstacle in the process is overcome; therefore it is helpful to know what is the rate of success after egg collection and embryonic transfer. We often publish success rates, so ask for our latest results.
2.Instra Cytoplasmic Sperm Injection (ICSI)
Intra Cytoplasmic Sperm Injection (ICSI) is a technique designed to assist fertilization using very small sperm. If the sperm level is low, either because of low numbers or because they do not swim well, or because they cannot get into the fluids around the egg, then reproduction may result. ICSI is successfully used to treat these conditions.
Who should receive this treatment?
Men have very low sperm counts
Men with low sperm count
Men produce sperm where there is no sperm (see Surgical Sperm Recovery)
Couples who have previously failed to obtain IVF fertilization, or when very few eggs are fertilized after IVF
What specific tests may be required before ICSI?
There are cases associated with severe sperm problems. These include chromosomal abnormalities, cystic fibrosis or hormonal imbalances.
Chromosomes carry genetic information from one generation to the next. They have the responsibility to determine not only what we look like but also how we feel about everything. We take blood from a man to check his chromosomes.
Cystic Fibrosis is a condition that affects the lungs and intestines and is also associated with deformities that cause male infertility. The probability that a healthy person may carry a risk factor for this condition is 1 in 20. For men with certain types of sperm problems, the risk is high. If both you and your partner have a risk factor then there is a 1 in 4 chance that your child may be affected. We take blood to check the condition.
Hormonal imbalances may be the cause of abnormal sperm production. Blood is taken to test this.
What does it involve?
The cycle is similar to IVF treatment.
This will depend on whether a new or frozen sample is used, or whether surgical removal is required.
What happens next?
ICSI treatment involves injecting one sperm directly into the center of the egg to aid fertilization. Once there, the nucleus from the sperm can meet the egg nucleus which completes the normal reproductive process. Fertilized eggs are inspected for fertilization. After two or more days, one or two embryos implant in your womb. Any other good quality embryos can be frozen and stored for future use.
How effective is it?
With regard to IVF treatment, success rates depend on a number of factors, but most importantly, women’s age. The chance does not improve as each obstacle in the treatment process is overcome, so it is helpful to know what the success rate after egg collection and embryonic transfer is. We often publish success rates, so ask for our latest results.
3.Compensated egg sharing
If you do not qualify for NHS funding, one way to help pay for IVF / ICSI treatment is egg sharing. To be an egg-laying person you must be physically fit and healthy, 18 to 35 years old, non-smoker, have no serious health problems, disability, genetic predisposition and do not gain too much weight in your height. To find out more about compensated egg sharing please contact us at Birmingham Women’s Fertility Center.
4.Treatment using donor sperm
Donor sperm are needed for a variety of reasons:
- When the couple’s cause of infertility is due to a lack of sperm, or a very small amount of sperm,
- it is not suitable for ICSI
- When a male partner has a genetic predisposition and a pre-implant genetic test is not appropriate For single and married women of the same sex.
Sperm donors are employed in a variety of domains. They need to be healthy and healthy, with no family history of genetic predisposition. Donors are tested for a variety of infectious diseases, including HIV and hepatitis, and certain genetic conditions, according to national guidelines.
We do our best to match the characteristics of the donor and the needs of the patient – nationality, skin, hair / eye color, height, etc.
Donor sperm can be used in conjunction with artificial insemination, IVF or ICSI.
5. Treatment with donor eggs
The fertility test we do includes a test at a cervical egg store. You will be given a follicle-stimulating hormone (FSH) test, which measures the amount of FSH in a blood sample, and in addition, you may be given an anti-mullerian hormone (AMH) test to test your egg store. If the egg shop is down, then donor egg treatment may be an option for you. We have a team of experts who will remove the complexity of donor egg treatment and guide you to consistent and effective treatment.
- The egg donation may be suitable for women who:
- You have had premature menopause
- You do not have ovaries or ovaries
- You have been treated for cancer of the ovaries
- It produces fewer or less eggs
- Be at high risk for transmitting genetic disease
What does this treatment mean?
Eggs from a donor are fertilized with the sperm of your partner or donor and transferred to your uterus.
Where do donors come from?
We appeal to egg donors, but there is a great need, and you may have to wait 18 months to find a suitable donor who matches your characteristics. This may be longer depending on your specific needs.
Alternatively, you could ask a friend or relative to donate their eggs, either directly or anonymously to someone else on the waiting list. You will then go to the top of the automated waiting list to find the next anonymous provider.
6.Test Tube Baby
Having a child and constructing a family is undeniably the vision of every couple. Unfortunately, there are many couples out there that still struggle to conceive even after many years of wedding. Test tube baby or IVF can assist you achieve your dream of becoming parents
To them we want to say you are not isolated, around 60-80 million couples face infertility every year worldwide, and about 15-20 million (25%) are in India.
Reasons can range from poor food selections and lifestyle choices to genetics and many more. If you find yourself in this situation, then there’s no need to be useless. Many couples have been where you are, and the majority have ended up conceiving and giving birth to healthy babies – thanks to medical breakthroughs like Test tube baby treatment or IVF – in vitro fertilization.
Owing to its high success rate, Test Tube Baby (or IVF) is one of the most suggested treatments for couples with infertility problems. In fact, a significant number of babies born in India every year come through IVF. This has made parenthood possible for many childless couples and has brought hope and faith for many struggling out there.
Unfortunately, the term “test-tube baby” is often misunderstood due to a lack of knowledge. Some believe that the process is artificial and may involve a different kind of ART procedure different from the conventional IVF treatment.
We hope to fix the confusion and answer all the questions you may have regarding Test Tube Baby.
Let’s start with the most significant item…
What is Test Tube Baby?
The first use of the term “test-tube baby” was in 1978. And this was after the birth of the first test-tube baby, Louise Joy Brown. Today, over 7 million healthy babies have been conceived and born via the test tube baby treatment process. As the name suggests, a test tube baby means the child is conceived outside the woman’s body.
To be more specific, the eggs and sperm are collected and fertilized in a laboratory. This process is formally known as IVF.
Surprisingly, in the early days when the test tube baby was rare, the lab instrument used was a test tube instead of a Petri dish, so the name might have actually been deceptive. However, today, the Petri dish is used for the process instead of the test tube. There is no difference between Test tube baby treatment and IVF.
IVF is generally known as Test tube baby.
It is worth noting that test tube baby treatment can also be done with donor sperm and eggs if the couple has problem with self egg or sperm.
Some partners may not be successful with the first cycle. In this case, they may need to go through two or three cycles. In this case, the cost of treatment will be extremely higher.
Test Tube Baby Process
Today, many clinics often adopt various techniques, but the core method for test tube baby treatment is the same. We discuss these steps below:
Step 1: Ovulation Stimulation / Super Ovulation
From the second day of menses, Gonadotropins or injections with the follicle-stimulating hormone are given. This results in the development of the follicles and the production of multiple eggs in the ovaries. Another injection of Antagonist is added to prevent spontaneous rupture of the eggs
A fertility specialist may do a scan and other tests required to monitor the process. To achieve the final maturity of the eggs, the woman is injected with HCG36.
Step 2: Egg Retrieval
The matured eggs are then retrieved through a minor method called “follicular aspiration” from both ovaries. During the egg retrieval process for test-tube baby, the woman is given some sedatives to prevent pain or discomfort. The number of egg varies. Generally, anything from 10-12 is considered good.
The couple may also decide to use Frozen or donor eggs.
Step 3: Semen Collection
Similarly, a semen sample is required, and the male partner will be asked to provide a sperm sample for further processes. Afterward, the sperm is “washed” to concentrate it. This needs to be done on the same day of Egg retrieval for the test tube baby treatment process. If the husband is not able to provide the semen sample, the same can be taken from TESTH.
Next, the retrieved eggs are mixed with the sperm in a Petri dish and then stored in a growth medium for at least 48 hours. An embryo is formed during this time.
In a situation where the sperm is less motile (low motility), a special technique known as ICSI is used to directly injects it into the egg.
The couple may also decide to use sperm collected through a testicular biopsy process if required and suggested by the doctor.
Who needs to opt for the Test tube baby process :
Some medical conditions that might instruct this method include:
- Failing to conceive after 1 yr or more of trying
- Older age
- Damaged fallopian tubes
- Endometriosis (usually in chronic cases)
- A problem with the womb (uterine cavity/ovaries) such as uterine fibroid
- Ovulation disorders (Irregular ovulation or no ovulation)
- Unexplained infertility
- Hormonal Imbalance
- Irregular Cycle
- Change in cycle duration
- Other health issues like PCOD / PCOS
- Low sperm motility or low sperm
- Nil sperm count
- Inability to have intercourse
- Erectile dysfunction
- Chromosome defects
- Infection such as gonorrhea, epididymitis, and orchitis
- Unexplained infertility
- Retrograde ejaculation
- Risk factors such as substance abuse and tobacco smoking
- Prior surgeries
- And other fertility problems
It is always suggested to consult with IVF and fertility experts to know the exact reason and further treatment process.
